Suddenly this week Wells Fargo is requring a notarized Name Affidavit form for every variation of the Buyer or Seller's name on any document they may have received (i.e., sales contract and any addenda, preapproval letters, proof of funds, earnest money checks, estimated HUD, loan documents, and Seller financial information).
I have 3 files suddenly being held up in Equator until we upload the Affidavits in a task called "Resubmit Documents"
From here on out I am going to try to encourage consistency on the front end ... but what a pain!

Just had one with this for a seller- luckily an easy fix as she is lives in the area and the title company drew up the form and notarized it. Now I have the form- thanks for sharing!
Terri- That each page that the seller(s) sign would have to be notarized is new to me. I understand that if the seller puts an initial for some signatures and then not for others or a shortened version of their name etc would be cause for concern.. The notarized Name Affidavit form would clear that up.
I had this notarized request happen the day before closing with an out-of-state seller back in March with WF. I have learned since then to be sure names match on everything exactly! Seller had included a middle initial in some places.

Thanks for providing this form here. Wells asked me for this on Equator today and IMHO it is a stall tactic...I'm ready to "toss" this negotiator!! ARGH. thanks again.

Wonder if you can upload the name affidavit form without the red marking? Wells is asking for name affidavit because my client has her maiden name on title while her bank statements has her married name.
I asked Wells to provide one, no reply thus far and the clock is ticking.
