This sounds very odd to me...if the 3rd person signed the Note, their name should appear on the approval letter. I do understand that the template they use likely only automatically pulls in the first two names, however, at the very least, the lender can manually add the third name. To be honest, I highly suspect the approval letter is initially generated by a template pull and put into a Word doc. Once there, it is completely able to be edited. Now, the person who generates that letter may be forbidden to edit the letter....but someone (supervisor) at the lender surely has the authority to amend the letter.
