On the last few that we have had with Chase they send the short sale affidavit / addendum with the closing package after reviewing the final HUD and it's customized with the seller's name the amounts that was discounted etc.

When I received it I wanted to make a copy so I would have it but it cant be changed as it was customized with the seller's information built in..

I used told to use the Fredie Mac addendum. it is on the Fredie Mac site
