I'm not sure this is actually true as we just received an approval a few weeks ago and it expires at the end of April.  We will need a least one extension on this approval but from what they are telling me is that they will not grant any extensions after the initial approval has been sent.  Has anyone run into this issue recently?  Is this something new?

I had this happen and they then foreclosed on the property.  Beware.
Did you receive ANY extensions?   And how long ago did this happen to you?
Yep ... it can happen. They take forever to do anything, but when they want you to perform it's another story altogether. The only thing you can do is inform them of the fact that you do need an extension, the reason why, and a date by which you absolutely, positively, can close the transaction. It may get you somewhere and it may not.
