New HAFA Document?

Hey Superstars.  Chase recently asked for another document for HAFA review for the seller...this document is entitled "Borrower Request for HAFA Consideration".  I (thought) I had all of the HAFA docs-sent in the RMA, Dodd-Frank, Hardship Affidavit, etc.  Is there a new document that I am not aware of?  Any info ya'll can provide is appreciated, as always.  I didn't see what I was looking for in the HAFA part of this site.

It's been a part of Chase's HAFA Borrower Solicitation Packet for awhile.

I sold my house a couple weeks ago and Chase did inform me that it was decided that an additional document would have to be submitted.  if memory serves me it looked just like the generic form that my agent had given me, but with a few more lines to complete.  Unfortunately I didn't receive my HAFA money.  Chase stated that they didn't receive my request even though I personally sent them everything, so I'm still working on that issue.  Do you know if a client has received HAFA money after the closing date?
