HELP !!! Does anyone have a RMA form for CMG mortgage !!!

I am the negotiator for a property that has CMG mortgage as the servicer. I have been trying to get an RMA form from them for over 7 months now.

My client had executrix documents from the courts. etc.

This bank is a nightmare has anyone dealt with them .. does anyone have a short sale form for them
