SPS is the 2nd lienholder. What forms do I need to send them in order for them to consider short sale?

Any info you have would be greatly appreciated.

First lienholder is Chase.

Hi Colleen - This reply is fairly delayed - hope you found your answers already!  They really didn't ask for anything special though when I dealt with them last year:

1-Hardship Letter

2-3rd Party Authorization

3-Listing Agreement

4-HUD-1

5-Purchase Agreement & all Associated docs (Addenda, Counters, etc...)

6-MLS Listing Printout

7-Supporting Comps & CMA - labeled at top of each page as 'Comps'.

Include loan # on each page.  This is verbatim what I've been asked for, and they were efficient & relatively quick to deal with - hope yours goes the same way.
