Initiate Wells Fargo Short-sale

I am in a process of initiating a short-sale with Wells Fargo and I need the following forms:

1. Short-sale affidavit (Is this the FHA short-sale affidavit?)
2. Borrower's assistance form

I have checked the Wells Fargo GRoup and I could not find this document#2. Does anyone has one and please send me a copy. Thanks.
