Getting MI info from a lender

I'm looking for a demand letter or some verbiage that I can use to request some information from BOA. Does anyone have anything or any success stories in getting MI info from a lender? 

-MI Company name

-MI Company contact information 

-Contact information for a person handling the file

-Certificate Number for the policy

-Loss amount on the deductible

    Ask your Sellers to get this info from their Lender re: MI company.  Many times, you can only get company name out of them, but that's a start..
