Chase short sale affidavit

Does anyone have a current copy of the Chase short sale affidavit?  I want to share it with a prospective buyer on a Chase short sale and make sure all parties are ok with it before I get too deep in the offer process.  It's the affidavit for Chase's own loans, not fannie, hud, etc.

    Just curious, why? Do you think there is an arms length/related parties/side deal with the seller/immediate flip planned?

      No, this client likes everything reviewed by his attorney before he signs documents.  I wanted to include it with the purchase agreement and our office disclosures.  If I thought there was an arm's length issue or 'flip', there would no need to go forward.
