Just heard back from the FC department. Here is what they told me...
"The auth just needs to be something signed by her. Doesn't have to be in any special form." Typical multiple-answer-to-the-same-question from a bank. Just to cover my bases I'm adding my logo to all LOAs from now on.

Normally they assign within 30 business days (i.e., 6 weeks). Once assigned, USBank negotiators are pretty good! Keep your chin up! ;-)

I am waiting for a US Bank Short Sale negotiator to be assigned for 60 days. I am doing the loan for the buyer and the Realtor is telling me they have NO loss mitigator assigned yet?? Seems like it's taking too long. Can anyone verify the approximate time to get loss mitigator assigned?

Thanks guys clarifying this to me. We do many deals and I am surprised I have not had an FHA yet... but I'm sure it's coming our way... :-)

Anna, the borrower usually knows, it's normally on the last monthly statement borrower got from lender, and it's even on our public record/taxroll info as to the type of purchase money loan when the bought. If all that fails, the servicer will tell us or we'll find out when they issue the HUD Pre-Foreclosure Short Sale Pre-Approval doc.

The homeowner should know if they used FHA or not. I had one that the servicer actually told me.
