NEED short sale package for PNC HELOC!

Hi all, I've just been told by PNC that they have a different short sale package/application when it's a HELOC as opposed to a 1st mortgage. PNC says they can't email or fax it because their system is down so they have to snail mail it to me! Does anyone have this app that they could send me today asap? Thanks!
