Midland Mortgage - Does anyone have their packet?

I'm meeting with some homeowners today who said Midland isn't that easy to work with.  I guess they tried to get a lawyer to work with them and Midland wouldn't accept the written authorization.  They want it on their forms.  Does anyone have the Midland Short sale Packet?  Would you be willing to email it to me? Or attache to the reply post?

    3rd Party Authorization
    Fax (405) 858-3000 (Authorization form is loan specific and you need to have homeowner call customer service and request that form be faxed/sent by mail).

    Midland Short Sale Package
    Homeowner needs to contact homeowner delinquency center and request a loan specific financial package- to be completed and signed by homeowner.
